Pequot Health Care ("PHC", "us" or "we"), including Pequot Pharmaceutical Network and Pequot Plus Health Benefit Services, respects the privacy of all visitors.  This Privacy Policy describes the types of information we collect on our Web site (www.pequothealthcare.com), our mobile sites, and our use of social media sites (collectively, the “Sites”), and any other personal information obtained when you call, email, or otherwise communicate with us.  In order to assist us in better serving our customers, we may collect certain anonymous information automatically, and may require certain personal information, including financial information, if you are making appointments or purchases or requesting information from us. This Privacy Policy outlines the type of information that PHC may collect and the manner in which such information will be used. Anonymous Information Automatically Collected

We may collect and use certain information about your use of this Web site that does not personally identify you. This information includes your IP address, MAC address, browser type, operating system, device-identifying information, the specific web pages visited during your connection, and the domain name from which you accessed the Sites. In addition, we may collect information about your browsing behavior, such as the date and time you visit the Sites, the areas or pages of the Sites that you visit, the amount of time you spend viewing the Sites, the number of times you return to the Sites and other clickstream data. A personal identification number provided by PHC may be required to access certain personal information. By providing personal information, you agree to be placed on marketing lists and may receive promotional materials from PHC from time to time.

Health Insurance Portability & Accountability Act (“HIPAA”) Compliance

Under HIPAA, protected health information (“PHI”) is individually identifiable information relating to the past, present, or future health status of an individual that is created, collected, or transmitted, or maintained by a HIPAA-covered entity in connection with the provision of, payment for, or use in healthcare. 

The collection, use, and disclosure of any PHI is subject to PHC’s Notice of Privacy Practices.  In the event of any conflict between this Privacy Policy and the Notice of Privacy Practices, the Notice of Privacy Practices will be followed.

Interest-Based Advertising

We may use third-party advertising companies that use tracking technologies to serve our advertisements across the Internet. These companies may collect information about your visits to the Sites and other websites and your interaction with our advertising and other communications. These advertising companies serve ads on behalf of us and others on non-affiliated sites, and some of those ads may be personalized, meaning that they are intended to be relevant to you based on information collected about your visits to the Sites and elsewhere over time. Age Policy

PHC has no intention of collecting any personal information (such as name, address, telephone number or email address) from individuals under 18 years of age. Any offers or promotions we conduct are not open to persons younger than 18 years of age. We reserve the right to verify age at any time for any reason and may reject a person who does not have valid age identification as requested by PHC . If a child has provided us with personal information, a parent or guardian of that child should contact us at the email address or phone number listed at the bottom of this Privacy Policy to identify the information that must be removed.

Sharing Information

The personal information you provide may be shared with other businesses owned or managed by the Mashantucket Pequot Tribe. These entities include, but are not limited to, the Mashantucket Pequot Gaming Enterprise d/b/a Foxwoods Resort Casino, The Spa at Norwich Inn, the Mashantucket Pequot Museum & Research Center, Lake of Isles, Mashantucket Pequot Interactive d/b/a Wondr Nation, and the Foxwoods El San Juan Casino. We do not anticipate that your personal information will be shared or divulged to any non-affiliated third parties by us, provided such disclosure may be necessary to: (a) comply with a court order or other legal process or other legal requirements of any governmental authorities, including but not limited to authorities which regulate gaming activities; (b) protect and defend the rights or property of PHC.

This Web site may contain links to other Web sites, such as social media sites like Facebook and Twitter, which may have privacy policies that differ from our own. We are not responsible for the privacy practices or the content of such sites.
